Goal: To increase the percentage of low income youth and adults who are engaging in proactive and preventive behaviors and services that will optimize health and wellness. Research has identified focus areas that impact the health goal: safe homes, healthy eating, physical activity, and access to health care.

 

Safe Home

Strategy:  Support individuals and families to reduce domestic violence.

Healthy Eating and Physical Activity

Strategy 1:  Increase the availability and affordability of nutritious foods.

Strategy 2:  Increase opportunities for physical activity.

Access to Health Care

Strategy 1:  Increase parents’ and young children’s use of developmental enrichment programs.

Strategy 2:  Assist individuals/families with enrollment and access to health care services, including home based services, and community health centers.

SUCCESS STORY:

With the support of the North Shore United Way, VNA Care Network & Hospice is able to literally help people get back on their feet.

Jim needed a hip replaced a second time because of infection. The 61-year-old, unemployed construction worker had very limited insurance and few resources to get through this difficult time. VNA Care Network & Hospice’s compassionate clinicians were there for Jim after surgery, helping him make the transition from hospital to home. The nursing care focused on getting Jim’s wound to heal properly and physical therapy on helping him regain mobility.

Support from the North Shore United Way enables VNA Care Network & Hospice to care for people of all ages during difficult times. The visits from skilled clinicians help people make the most of their health and continue living independently.
